Specific Responsibilities and Performance standards for ConnectCare Worker:

Personal Conduct and Appearance:

Maintains regular office hours as assigned by the office manager.

Maintains neat and appropriate dress and appearance.

Displays efficient work habits and encourages the same in other employees.

Maintains moral conduct both at and away from the office which reflects well on the agency

Complies with personnel policies

Performance Standard:  Minimal absenteeism, efficient use of time, appearance and activity appropriately professional.

General Office Duties:

Receive and handles clients who are interested in the CFS ConnectCare program and direct to appropriate placement or community resources.

Performance Standard:  When needed clients are appropriately greeted, effective flow of phone contacts, assist clients in a professional manner. 

ConnectCare Worker:

Ability to do case management tasks. Management tasks include frequent phone, email, text communication between clients and available community resources. Management tasks also include placing children from parents care to foster parents care, noting all attention to child’s needs and necessary legal documentation, including visits with parent. This entails high level of skill to interact with at risk clients, mothers and fathers, their children, and possibly with other family members—all with the intent to collaborate and problem solve regularly for holistic family needs and goals.

Ability to collaborate with and facilitate professional team approach with foster parents, connected with each client and their children.

Offer education and training regarding child development and parenting strategies for clients and foster parents as needed.

Capability to follow up with medical, emotional and behavioral, educational, needs for the child (possibly the mother and immediate family) with appropriate evaluations and resources to instill overall wellbeing.

Capacity to collaborate with the Children’s Division and any open cases or investigations.

Initiate and maintain foster parent license and tasks to comply with the state of Missouri.

Religious Requirement:  A person who is an active, faithful member of a church who is willing to sign a statement of faith and whose life and conduct has been such as to receive a favorable recommendation from the eldership of a local congregation.

Education and Training:  A minimum of a Bachelor’s degree in social work or related field.
