This clinical social worker position works with the Positive Health Access to Services and Treatment (PHAST) team.
The scope of services of the PHAST program includes rapid linkage, HIV treatment, and sustained engagement in primary medical care through intensive nursing case management, outreach services, social services, and
coordination of care with appropriate community agencies. For newly diagnosed HIV positive individuals coming either from the ZSFG campus or community testing sites, the PHAST/RAPID team provides HIV test disclosure,
post-test counseling and education, partner services, and immediate access to medical care and HIV treatments, often on the same day of diagnosis and no later than 5 days from diagnosis. Incumbent will successfully identify
patients for inclusion in the PHAST/RAPID program, will visit patients in a variety of settings, will assess their
psycho-social needs, and assist them to utilize the primary care system. Incumbent's engagement with patients will
result in increased use of primary care services, decreased use of urgent and emergency care services, and
increased adherence to prescribed plan of care.
Incumbent will coordinate with members of the PHAST/RAPID team and the inpatient HIV Consult Service as a
liaison with the outpatient HIV medical clinic, the Emergency Department social services staff, and other key
community primary care clinics and organizations. Under the direction of the Clinical Social Work Supervisor, the
incumbent is responsible for social work efforts to identify those persons infected with HIV who do not currently
have a source of primary medical care and social services and who require intensive assistance in gaining access
to care. Incumbent will facilitate engagement in care by coordinating referrals for patients with Navigators who can
accompany identified individuals to appointments and by identifying other mechanisms for improving adherence.
Incumbent will also have responsibilities coordinating key collaborations within the DPH, coordinate training and
technical assistance with other clinical sites, and participate in the Getting to Zero RAPID committees.
Under the direction from the supervising social worker, the incumbent may also work with HIV affected women,
families, and other populations on an on-going basis within the Women's Center of Excellence (COE) at Ward 86 to
provide them with comprehensive clinical social work services. These services include: psychosocial assessment,
mental health and substance abuse assessment and treatment referral, crisis intervention services, HIV disclosure and prevention counseling, and short-term therapy as needed. Additional responsibilities may also include health
education, child development and parenting education, benefits advocacy, referral and linkage to psychiatry,
community mental health services, residential substance abuse programs, case management, and other
community-based agencies and services as appropriate. The incumbent will work closely with all members of Ward
86 Women's Clinic as well as community clinics and the jail to ensure smooth and timely transition into primary care
for women with HIV. The incumbent may also be required to facilitate pre-clinic Women's COE weekly case
conference, participate in social work meetings, staff meetings and will provide consultation to medical and social
service providers.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% - 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.
