The Eugene campus of the Child Development and Rehabilitation Center provides interdisciplinary clinical services for persons with developmental disabilities and other special health care needs. CDRC Eugene programs and services are provided at the Clinical Services Building on the University of Oregon campus. Note: This position is located in Eugene, Oregon Conducts mental health and psychosocial assessments with patients and their families as part of an interdisciplinary team and/or part of the social work clinic providing evaluation and diagnosis. The LCSW will identify and assess the mental health, psychosocial and cultural conditions of patients and their families in regards to patient functioning capacity and support systems available for treatment planning and may also provide treatment. Participates in treatment team meetings with various health care staff to provide consultation and/or psychosocial assessments that are used in establishing treatment goals, resources and referrals. As appropriate, makes selective visits to patient homes, schools, and social service agencies. Refers patients/family to community resources for assistance (financial, vocational, schools, transportation, insurance, housing, medication) and assists family in gaining access to resources. Develops treatment plans emphasizing continuity of care, appropriate placement, and maximum use of appropriate resources. Advocates for the patient within the medical setting and community. Assists clients and families in obtaining and maintaining health insurance. Provides assistance in resolving issues with health insurance payment and coverage. Provides crisis, short- and long- term counseling to patients and/or their families, as necessitated. Provides supportive counseling to reinforce patients' strengths and assist them in dealing with the stress and feelings of grief, fear, and anger associated with acute and chronic illness, injury, hospitalization, or disability. May facilitate group therapy/sessions to provide information to patients and their families around social and emotional development.
