The primary goal of the Service Coordinator is to support families promoting their child’s development and to facilitate the child participation in Early Intervention activities. The Service Coordinator builds a collaborative relationship between families and providers requiring open dialogue to develop, implement, monitor and modify Early Intervention goals and strategies.
Service Coordinators provide a central point of contact for families who have been referred to Early Intervention. Service Coordinators assist families within a designated community with identifying and meeting each child’s individual developmental needs through coordination of informal and formal supports.
Good organization, time management and computer skills are highly recommended for this position.
Service Coordinators will conduct home visits in accordance with the referral to the Early Intervention (EI) program and maintain extensive documentation to comply with state regulations.
Assisting parents of infants and toddlers with gaining access to, and coordinating the provision of early intervention services required. This includes completing intake assessment, coordination of initial evaluation and completion of Individualized Family Service Plan as needed.
Advising families of their rights and procedural safeguards and obtaining appropriate consent.
Coordinating, facilitating and monitoring the delivery of services in a timely manner.
Helping infants and families access necessary treatment and supports from qualified providers and making appropriate referrals to community resources.

The educational requirement is a minimum of Bachelor’s degree in child development, social work/social services, psychology or related human services field. Personal vehicle for travel to designated region of service required. Valid driver’s license and vehicle insurance required. Good organizational, time management and computer skills are highly recommended for this position. Bilingual (Spanish-English) preferred.
