This position is part of the home visiting team, which uses the Parents as Teachers (PAT) curriculum, an evidence-based model which provides positive parenting support through comprehensive home-based early childhood services including parent coaching, case management and center-based services to pregnant and parenting youth and their children. S/he will collaborate with the other home visitors and Doulas to provide comprehensive services in an integrated program as well as the rest of the staff to provide seamless high quality services. This position is located in Austin and serves several communities on the West side of Chicago. Salary: $38k.
Maintains a full weighted caseload based on service intensity of participants and their children.
Assists with recruiting, screening, and intake of participants when directed.
Conducts home based visits based on assigned service intensity with the participant that consist of age-appropriate parent-child activities and observations intended to enhance the parent-child relationship as well as service planning, life skills and emotional support through a trauma informed approach.
Develops written service plans, with regular updates, for each assigned case including participant and children, specifying goals to be achieved and services to be provided.
Conducts home based visits with the participant that consists of approved, age appropriate parent child activities and observations intended to enhance the parent child relationship. Service needs, referrals and goals progression are also discussed during the visit. Performs crisis prevention and intervention as needed. Assists with transportation of participant and children as required and approved by supervisor.
Works in tandem with other New Moms staff to integrate services across the Agency; collaborating with staff and other collateral sources on specific case-related issues.
Maintains working relations with other social service or public agencies; providing linkage of participants to appropriate resources and advocates for participants as necessary. Secures appropriate release and consent forms and facilities inter-agency referrals as needed.
Assists with weekly parent support group meetings as needed.
Performs written and electronic recordkeeping functions as required and in a timely manner.
Works to consistently enforce program compliance under the direction of the program manager, working to maximize participant retention as well as program safety.
Coordinates discharge planning, resources and referrals.
Actively participates in weekly supervision, team meetings, and agency staff meetings.
Regularly participates in agency or professional group seminars or conferences to enhance professional knowledge and job skills.

The successful candidate will possess:
Enthusiasm for mission-driven work and interest in work with young moms at the time of pregnancy and early parenting.
Master’s Degree in the area of social work, child development counseling, or psychology preferred; bachelor’s required.
Parents as Teachers (PAT) certification preferred.
Two or more years of case management and crisis intervention experience working with vulnerable populations, preferably with early childhood through adolescent parenting, child/family development and urban issues.
Endorses a Family-Centered, Trauma-Informed, Strengths-Based practice model that integrates harm reduction, motivational interviewing, executive skills, infant mental health, and positive youth development practices in working with young moms and their children required.
Position requires utmost integrity; ethical behavior and appropriate business practices are expected.
Strong interpersonal and communications skills, and ability to work both collaboratively and independently.
A functional car with valid insurance, along with a valid driver’s license.
