The Family Support Coordinator provides content area support and oversight in the areas of ERSEA (Eligibility, Recruitment, Selection, Enrollment, and Attendance), PFCE (Parent, Family and Community Engagement), Health and Nutrition, Inclusion, and Social Emotional to the Easterseals Head Start/Early Head Start/Preschool for All/Prevention Initiative programs. The Family and Health Coordinator provides ongoing supervision to assigned Family and Health Advocates.
Planning and Program Services:
Develop and maintain professional relationships, and work with the Family and Health Advocates, other Easterseals staff, partner site directors and staff, and the community to ensure consistent, high quality parent engagement, family support, health and nutrition, inclusion, and social emotional services at all assigned HS/EHS/PFA/PI programs.
Work with Easterseals staff, site directors and staff, and the community to ensure that programs recruit and enroll HS/EHS/PFA/PI children, including children with disabilities, to meet their funded enrollment numbers and to keep an active waiting list.
Provide support to assigned Family and Health Advocate supervisees at sites on a regular basis (complete at least 2-3 site visits each week), including assisting with building collaborative relationships with the Education and Inclusion team, Program Operations team, and site directors and staff, in order to provide integrated services to families.
Ensure that a family strengths-based approach is understood and followed by staff.
Ensure through ongoing individualized supervision, support, and ongoing monitoring that FHAs are fulfilling their position responsibilities, including: building trusting relationships with families; providing ongoing support through significant monthly contact with each family according to the requirements of each program; family needs assessment and goal-setting; referrals and services, follow-up, and advocacy to meet family needs and interests; implementation of Baby TALK parent engagement curriculum; implementation of attendance follow-up procedures; ensuring that enrolled children remain up-to-date on health requirements; scheduling and facilitating Screening Review Team and Social Emotional Review Team meetings; supporting families through the IEP and IFSP referral process.
Ensure that FHA (II)’s are meeting with each family enrolled in PI in their home and/or community location twice per month for one hour utilizing the Baby TALK home visit model; are utilizing the PICCOLO assessment tool and the Baby TALK model to support parent-child interactions; planning and facilitating at least one group parent-child socialization activity per month; are implementing Baby TALK to fidelity.
For FHA (II)’s, conduct observations of home visits and parent-child group socializations twice per year and as needed.
Participate on the Parent, Family and Community Engagement (PFCE) team to develop and implement a PFCE Plan that supports meaningful parent and family engagement outcomes.
Work with Easterseals and site staff to promote parent engagement, including through parent participation on the site Parent Committees and the agency Policy Council.
Ensure the provision of case management and referral for social services to HS/EHS/PFA/PI families in cooperation with other social services, health and mental health agencies and providers through developing and maintaining community linkage agreements.
Participate in the community assessment.
Participate in annual strategic planning.
Participate in the School Readiness team and the development and implementation of the School Readiness Plan as part of an integrated leadership team.
Participate in active supervision to ensure the safety of all children and that no child is ever left alone or unsupervised.
Participate in program planning and implementation of all service areas as part of an integrated leadership team.
Record Keeping, Reporting and Communication:
Implement systems and monitor individual supervisees’ work to ensure that all necessary information, including waiting list, enrollment, Family Partnership Agreement strengths and needs assessments and goals, referrals, follow-up and case notes, is entered into COPA and child/family files in a timely manner.
Implement systems and monitor individual supervisees’ work to ensure that health histories, physicals, and dentals are routed to program aides for COPA entry.
Prepare monthly reports.
Facilitate and/or participate in regularly scheduled content area, team, director, and other meetings.
Complete the annual PIR as part of the leadership team.
Ongoing Monitoring and Self-Assessment:
Implement monitoring systems for ERSEA, PFCE, Health/Nutrition, Inclusion/Social-Emotional.
Complete site verification reports for all site visits, and review those submitted by supervisees.
Participate in roundtables, and in the planning, implementation, and follow-up for site support plans and performance improvement plans.
Participate in the ongoing self-assessment of the program.
Implement reflective and relationship-based practices.
Meet with each supervisee monthly for reflective supervision.
Provide ongoing technical support to the Family and Health Advocates, including their performance evaluations and professional development plans.
Provide individual training follow-up to supervisees as needed.
Conduct training follow-up with individuals and small groups.
Participate in the development of an annual professional development conference.
All other duties as assigned.
Bachelor’s degree or higher in social work, human services, family services, counseling, or a related field (Masters preferred); or, bachelor’s degree in another field with the following to be earned within 18 months: Illinois Gateways to Opportunity Family Specialist Credential.
Home Based CDA, Infant Toddler CDA, or 12 credit hours in EC or child development; or, Home Based CDA to be earned within 24 months.
1 year of supervisory or mentoring experience and/or 2 years of home visiting experience (both preferred)
3 years of direct service experience.
2 years of experience in Head Start/Early Head Start.
Head Start Performance Standards.
Valid driver’s license.
Travel to Easterseals and partner sites regularly.
