Erie Family Health Center is looking for a responsible, self-motivated, team player like you to join our Case Management Team! The Case Manager Team Leader works with case managers, medical providers and nursing staff to identify potential clients, assess, and deliver supportive services to clients.
Conduct outreach, advocacy, assessments, care plan, and home visitation
Maintain an active caseload and conducts outreach to out - of -care patients
Follow the Department of Human Services Maternal and child code requirements
Act as a mandated reporter to the Department of Children and Family Services
Monitors and facilitates referral and follow-up process according to care plan and risk assessment
Conducts bi-weekly Quality Assurance to measure performance and 1:1 meetings with Case Manager
Provide leadership support and oversight to direct reports
Bachelor's degree social work, child development, community health, or Master of Social Work or Master in Community Health or related field
A minimum of 1-2 years' experience working with the target population and/or a minimum of 1-2 years' experience as a Team Leader in a related field
Bilingual (English/Spanish) required (May be required to translate for non-English/Spanish only speaking patients and/or guests.)
Local travel, valid Illinois driver's license is also required
Cultural competency to work in a low income, minority environment
Word-processing or data-entry skills
Ability to establish and maintain effective working relationships with co-workers, clients and community partners.
Ability to plan, organize, implement, evaluate, and modify program needs
Ability to problem solve and address issue as they arise
Ability to maintain confidentiality and trust of clients
