Full Time 40 hours Grade 006 Social Work Division SMH Responsibilities THIS POSITION HAS AN END DATE OF 2 YEARS FROM DATE OF HIRE. General Responsibilities: The Community Health Worker/Recruiter provides outreach supportive, concrete and case management services to adolescents and women of reproductive years and their families in an effort to improve their health, personal well-being and parenting skills. The Community Health Worker provides services in collaboration with other members of the Building Healthy Children team. Specific Responsibilities: Clinical Services: The community health worker (CHW) is assigned to pregnant or parenting teens/women and their families by the project social worker The CHW assumes the following responsibilities: (1) establishment of a supportive relationship with assigned families; (2) in conjunction with clients, identification of a family service plan; (3) case management; (4) assistance to the families in acquiring tangible needs (housing, food, insurance, household goods and baby items); (5) facilitation of participants attendance at health, other collaborating partners services and social service appointments; (6) promotion of personal and child health and safety including exercising sound judgment in recognizing critical risk situations and based on training respond to risks (7) help in securing quality child care services 8) support during personal crisis and (9) facilitation of and encouragement in the pursuit of educational, employment and other personal goals. The CHW intervention occurs primarily in the participantâ™s home, through completion of regular home visits. The CHW is available to directly transport program participants to medical, behavioral health or social service appointments, program activities or to other locations that are part of the participantâ™s service goals. In addition, the CHW is available to clients during crisis. The CHW assists in orienting consumers and the community at large to the project. The CHW works collaboratively with other members of the project and health care team and with other community service providers. Interventions are documented according to Project and Social Work Division policies and procedures. The CHW provide clinical coverage in the absence of their co-workers. Administrative: The CHW participates in activities of the Social Work Division including, but not limited to, staff meetings, mandated Hospital and Social Work Division training, UHS review, time reporting, and documentation of clinical activities. Qualifications The CHW must have a high school education or equivalent accomplishment. In addition, the CHW must demonstrate good verbal and written communication skills. Desired qualities include experience in a service related field preferably in early child development or parenting support services, demonstration of empathy and acceptance of a variety of families, and the ability to work one-to-one in a supportive, nurturing relationship. Willingness to work within the community and to transport clients is required. The CHW must have a registered vehicle and requisite insurance as well as a current driving record that reflects the UR safer driver program in order to transport clients. Fluency in Spanish is desired.
