Responsible for case management of mental health and provider services to about 10 EHS providers per year to be conducted at provider sites (as needed) according to assignment, within agency objectives, policies, and program.
Conduct site visits every month using the comprehensive areas lens to ensure compliance and that individualization is evident for children.
Establishes and maintains clear communication channels between the external partners, parent educator, family supports specialist, providers, and coordinators, managers, and EHS/HS teams.
Works with the team to plan and implement a comprehensive child development program that meets the HS/EHS grant requirements.
Inspects Day Care homes for health, safety, fire, and occupancy violation for the purpose recommending necessary corrective action to assure environmental quality.
Support EHS team as needed.
Provides individual and group mental health counseling services for early childhood and families.
Conduct group mental health sessions.
Develops diagnostic assessments and treatment plans with providers and provider clients.
Maintains participant family and providers records as required by the agency and program standards.
Responsible for working with external healthcare partners to secure on-going access for providers for the children being served in their family child care home program.
Responsible for updating all forms and documentation related to providing services in the areas of disabilities, mental health, nutrition, and health.
Compiles and enters statistical data as required by the Program and agency.
Provides data and information that will assist in program reporting.
Collaborates with personnel from relevant agencies, DFSS, HS/EHS, communities toward implementing services.
Manages all agency expectations for case recording, statistical and other routine forms with accuracy, efficiency, and punctuality.
Attends and participates actively in meetings for clinical and/or full staff.
Participates in on-going training and staff development provided by DFSS, EHS, and SGA, workshops and conferences.
